Hyperhidrosis Surgical Treatments
Surgical Treatment for Hyperhidrosis
Although alternative treatments such as oral medication, BOTOX®, Drysol® and Drionic® are available, the only long-lasting, effective treatment is to surgically stop the signal transmissions of the sympathetic nerve impulse to sweat glands. Basically, this can be achieved for all locations in the body such as palms, face, armpits, and feet. This procedure is known as Endoscopic Thoracoscopic Sympathectomy (ETS)
The surgrical procedure and technique
The best person to perform Endoscopic Thoracic Sympathectomy (ETS) is a highly trained, experienced, thoracic surgeon.
The operation is performed on an outpatient basis while patient is under general anesthesia administered by a board-certified or board-eligible anesthesiologist. The sympathetic nerves are located along the back, just behind the ribs. The surgeon uses a scope with magnification and illumination provided by the camera to view the sympathetic nerves. The vascular surgeon cuts or clamps the sympathetic nerves of the ganglion through two small incisions (5 to 10 mm) below the armpit area on each side of the chest. In the cutting method, the nerve is simply cut. These nerves are cut to stop or reduce the body's ability to produce sweat in those identified problem areas. Our surgeons prefer this method over the clamping method. In the clamping method, metal clamps are applied on the nerve to stop the sweating from a particular section of the nerve; the success rate for this method is low. The procedure is performed bilaterally in the same session. After patients wake up from the anesthesia, they are moved to a recovery room, where they are carefully monitored, before being discharged to go home. Patients can return to work or school within several days. The procedure is extremely effective for palmar and axillary hyperhidrosis. The endoscopic technique is very safe and is curative in 98% of patients.
Results The primary indication for surgery was palmar hyperhidrosis (PH) in 302 of 309 patients (97.7%), although in 7 patients (2.3%) axillary hyperhidrosis (AH) was the primary indication. A family history was elicited in 74 of 132 (56.01%) and a provocative response to hand lotion was present in 101 of 132 (76.5%). Thoracoscopic sympathectomy afforded almost instantaneous cures for PH, with marked improvement in 100% for whom the sympathectomy was done. Of 180 patients prospectively questioned in detail, 173 (96.1%) had some degree of plantar hyperhidrosis. Of these, 148 (84.4%) had some improvement, with 70 (40.5%) achieving complete relief of the plantar hyperhidrosis. In 98 patients who had some complaints of AH, 68 (69.4%) were completely relieved with the AH, while 25 (25.5%) were relieved but not completely cured. In 7 patients, the primary indication for sympathectomy was AH and of these, 3 (42.9%) had complete relief, 2 (28.6%) had partial relief, and 2 (28.6%) had no relief. Of the entire series of 309 patients, 4 (1.3%) developed severe compensatory hyperhidrosis (CH). In 180 prospectively questioned patients, CH was present in 81 (45%).

Excessive Sweating

Cure Sweaty Hands - Cure Sweaty Feet -Cure Sweaty Underarms -Cure Excessive Sweating - Cure Hyperhidrosis - Cure Facial Blushing.

What is excessive sweating?Excessive sweating, also called hyperhidrosis, can affect the entire body, but usually occurs in the palms, soles, armpits, and/or groin area. Excessive sweating is normal when a person is anxious or has a fever. However, when the condition is chronic, it may signal thyroid problems, low blood sugar, nervous system disorders, or other medical problems. What are the symptoms of excessive sweating?Areas that produce excessive sweat usually appear pink or white, but, in severe cases, may appear cracked, scaly, and soft (especially on the feet). Other symptoms may include a bad odor caused by bacteria and yeast in the wet skin. The symptoms of excessive sweating may resemble other medical conditions. Always consult your physician for a diagnosis.Treatment for excessive sweating:Specific treatment for excessive sweating will be determined by your physician, based on:your age, overall health, and medical history severity of the condition cause of the condition your tolerance of specific medications, procedures, or therapies expectations for the course of the condition your opinion or preference Treatment may include topical, oral, surgical, or nonsurgical treatments:methenamine solution applications to the area (to control heavy sweating) nighttime applications of aluminum chloride solution to the area (to control heavy sweating) thoracoscopic sympathectomy - a surgical interruption of the sympathetic nerve pathways that lead to the sweat glands botulinum toxin A injection (Botox®) - botulinum toxin helps to inhibit the release of acetylcholine (a substance that is active in the transmissions of nerve impulses) tap water iontophoresis (applying a weak electrical current to the area) psychological counseling and/or medication (to reduce anxiety)

What is the probable cause of the excessive sweating?
Definition
Hyperhidrosis is sweating more than would be expected in the temperature of the environment.
Description
This condition is characterized by constitutional hyperactivity of the eccrine sweat glands. The disorder may be generalized, consisting of excessive body sweating, or localized, with sweating confined to the palms, soles, armpits, groin, and under the breasts. As a rule, onset is in childhood or during puberty. Patients experience a heightened reaction to sweating stimuli such as anxiety, pain, exercise, tension, caffeine, and nicotine. The sweat-prone areas may be localized or generalized. When the palms and soles are involved, the skin may appear pink or blue-white, and may even macerate, crack, or scale, particularly on the feet. Patients often experience spontaneous relief in adult life.
Causes
The cause is unknown. In very rare cases, hyperhidrosis of the palms and soles is thought to be inherited as an autosomal dominant genetic trait. One must distinguish between idiopathic hyperhidrosis from excess sweating due to malfunction of the thyroid or pituitary gland, infection, diabetes mellitus, tumors, gout, and menopause. The disorder affects males and females in equal numbers.
TreatmentBefore treating generalized hyperhidrosis, a possible primary disorder must be ruled out. For patients with palmar-plantar-type hyperhidrosis, cotton socks and shoes that promote the circulation of air prevent overheating of the feet. Alternating footwear is helpful. Applications of medicated powder formulated to hamper bacterial growth is useful. For refractory cases, topical agents such as aluminum chloride in ethyl alcohol may be indicated for axillary sweating but is often useless for sweating hands. Short-term courses of anticholinergic drugs are also useful in severely afflicted patients but the side effects of dry mouth, drowsiness and constipation frequently occur. Super-antiperspirants may be of some help. These are essentially superstrength formulas of regular underarm antiperspirants. The active ingredient, aluminum chloride, actually reduces the sweat output (unlike deodorants, which just deal with odor). Heavyweight formulas are available over the counter in concentrations up to 12 percent, compared with 4 to 6 percent in regular antiperspirants. Even stronger concentrations are available with a prescription. The trick is to use them correctly. They work only when applied to dry skin. Do not apply them to broken skin or freshly shaved underarms. To increase their effectiveness, apply them at night before bed, since the nervous system is less active during sleep. In the morning, shower as usual, then apply regular antiperspirant to the underarms. Two or three applications of this combined treatment should keep one dry for another three days. One approach to treatment is to control stress. Whether or not emotional stress is the instigator, stress does make the sweating worse. Stress management therapists take three main approaches to help a patient calm overactive sweat glands. First is the daily use of relaxation tapes or meditation. Second is biofeedback training; and third is traditional psychotherapy that investigates and aims to remove the causes of stress. Surgery is available in extreme cases in which sweat glands are removed from underarms, or the nerves that trigger the sweat glands in the hands can be cut.

UCLA Study Finds Evidence That "Sweaty Palms" Syndrome Is Genetic And Underreported
Source: University of California - Los AngelesMarch 6, 2002 Imagine being afraid to shake someone's hand, or to simply hold hands with a sweetheart. Beyond just embarrassing, "sweaty palms" syndrome is often a debilitating disorder that can affect one's work and life.
A new UCLA study in the February issue of the Journal of Vascular Surgery shows strong evidence that sweaty palms syndrome is genetic. It may be caused by a dominant gene - indicating that family members of those who have the disorder may suffer from it more than has been previously reported.
"Traditionally, this syndrome was thought of as stress-related and has not been taken seriously by the medical community," said Dr. Samuel S. Ahn, principal investigator and professor, UCLA Division of Vascular Surgery. "This is one of the first studies helping to support that 'sweaty palms' is a real physiological disorder that can be passed from generation to generation."
According to Ahn and his collaborators in the UCLA Department of Human Genetics, the study indicates that as much as 5 percent of the population may be at risk for some form of hyperhidrosis, commonly known as sweaty palms syndrome, which causes excessive sweating, most often in the hands and feet. Less than 1 percent of the population was previously thought to be affected.
"Hyperhidrosis can truly affect one's life and career, such as a police officer dropping a gun and having a suspect literally slip away, or a fireman not being able to pull a hose or a banker unable to handle money due to severely sweating palms," Ahn said.
UCLA researchers took detailed family histories from 49 patients with hyperhidrosis and found that two-thirds (65 percent) reported family recurrence of the disorder, compared with zero percent in the control group.
Although the disorder appears to be inherited in a dominant fashion, the possible genes involved may not always cause hyperhidrosis. If one parent has the disorder, the study found that children have a 28 percent risk of also having hyperhidrosis, whereas the risk would be 50 percent if the gene produces the disorder directly. This indicates that other genes may also be necessary for hyperhidrosis to develop. If a child has the disorder, 14 percent of parents have it too.
"The strong inheritance pattern and large number of people with family recurrence of the disorder indicate that hyperhidrosis may be caused by a dominant gene," Ahn said. He adds that the disorder does not appear to be related to sex or ethnicity.
The next step, according to Ahn, is to test the DNA of people with hyperhidrosis and begin the process to of trying to identify genes that cause the problem.
Ahn's interest in pursuing this study began when a former patient of his told him that her six-week old infant also had hyperhidrosis. Ahn then realized the possibility that hyperhidrosis may be inherited and not environmentally related to stress.
Hyperhidrosis is caused by the sympathetic nerve, which governs the nervous system's "fight or flight" response. The sympathetic nerve causes blood vessel contraction in the hands and/or feet, leaving the extremities cold and sweaty. In people with hyperhidrosis, the perspiration is often excessive and continuous.
Treatment for hyperhidrosis of the hands now includes a minimally invasive surgery procedure, thorascopic sympathectomy, where a surgeon will snip the sympathetic nerve connected to the hands. Since the sympathetic nerve is not involved in motor skills or sensation, says Ahn - who is a pioneer of the procedure - the surgery simply stops the ability of the nerve to create hyperhidrosis. The procedure at UCLA has been 100 percent successful.
The study was funded by the California Vascular Research FoundationMORE ARTICLES

Less Invasive Procedure Can Now Cure Sweaty Palms

By M. Mary ConroyCHICAGO -- A minimally invasive procedure, developed by interventional radiologists in Belgium, cures patients who have excessive sweating of the palms without the need for surgery, according to results reported at the Radiological Society of North American annual meeting. The treatment for the condition, also known as palmar hyperhidrosis, involves blockage of the nerve, referred to as sympathectomy, to stop the excessive sweating.Forty-seven of 50 patients were treated successfully on the first attempt and the remaining three achieved dry palms with a repeat procedure, said lead investigator Dr. Hugues Brat, head of the radiology department at Center Hospitalier Hornu-Frameries.Guided by CT fluoroscopy, Brat places a highly flexible needle at the third vertebra and threads it into the sympathetic junction, after saline is injected to avoid the accumulation of fluid around the lungs. He then injects phenol to "kill the nerve and cure the patient." Patients require two procedures, one for each hand. The procedure takes about 20 minutes and is done under local anesthesia. In this series 16 patients had side effects such as chest pain, but the symptoms resolved in a few hours. Before this technique, called percutaneous sympathectomy, was developed, surgical sympathectomy was used to treat this condition, which requires an open chest operation and a hospital stay of several days, Brat pointed out.Typically, the surgery costs about $5,000 for each side and has a potential risk of bleeding and partial paralysis. The estimated cost for percutaneous sympathectomy is $1,000, he said.Brat said he and his colleagues have already performed more than 300 procedures and "now we have some patients who we have followed for more than 4 years, so we know this works."Currently the procedure is done at his institution and a second center in Belgium, but he said that his team has been teaching the technique in a series of workshops and they plan to launch an online training program early next year.

Why Do I sweat So Much ?

Cure Sweaty Hands - Cure Sweaty Feet -Cure Sweaty Underarms -Cure Excessive Sweating - Cure Hyperhidrosis - Cure Facial Blushing.

You have to get up in front of the class to make a presentation. As you walk to the front of the room, you feel your shirt sticking to your back and your armpits. You're sweating up a storm. Although it may seem like you have some strange disorder, it's actually perfectly normal to sweat, not just when you're hot, but also when you're nervous. Emotions can affect the sweat glands, too.
Sweating is one part of puberty. When your body starts to change, your 3 million sweat glands become more active. At the same time, glands in your armpits and groin and on the palms of your hands and soles of your feet produce oilier sweat, which has an adult odor. Sweating plays an important role in the body because it helps maintain body temperature by cooling you down. When you're hot and you sweat, that moisture evaporates and cools you off a bit.
So how should you handle sweat? Take a bath or shower daily, and if you're worried about your smell, use a deodorant or a deodorant with antiperspirant (a deodorant masks odor, whereas a deodorant combined with antiperspirant prevents sweat).
It can also help to wear clothes made of natural fibers, such as cotton or linen, especially in the summer heat. Pads called underarm shields or dress shields can also help absorb sweat and prevent embarrassing underarm stains. These pads attach to the armpit area inside a person's clothes where they absorb sweat. You can buy them in the lingerie departments of many department stores and at some specialized sports stores. Some teens also keep an extra shirt in their lockers so they can change at school. If you still worry about your sweating, talk to a doctor. Stronger antiperspirants are now available with a doctor's prescription - your doctor may think a prescription-strength antiperspirant might help you.
